The form will have the enctype atribute to "multipart/form-data" and the input should have "file"  type.

   <form name="newad" method="post" enctype="multipart/form-data"  action="">
 <table>
 	<tr><td><input type="file" name="image" ></td></tr>
 	<tr><td><input name="Submit" type="submit" value="Upload image"></td></tr>
 </table>	
 </form>
